Put a requirements section in each document that is used in some fashion to verify a requirement (for example, include a requirements section in test plans test procedures and test reports. It is best to a requirement right into the section(s) in a test procedure in which it will be verified. It would be best to create a verification plan document that describes exactly how each requirement will be verified before any verification work begins.
