A case management coordinator, home care is the person who supervises and directs home health care aides that visit patients who need help in their homes with nursing, light tasks, and/or maintenance duties. The coordinator typically helps to ensure that properly credentialed care personnel are working with patients. He or she is typically the point person that will handle any complaints or concerns regarding care, and this person works to help resolve these issues with the patients and caregivers.
