Senior medical transcriptionists transcribe and interpret dictations made by health care professionals (such as physicians) regarding patient assessment, diagnosis and prognosis, and therapeutic procedures, among other areas. These transcriptions require a high level of skill or accuracy. Reports are transcribed in an electronic format to create files that represent the full medical history of a patient.
