Job Description for Disability Case Manager

A disability case manager is primarily responsible for assisting and supporting patients with disabilities, as well as choosing health care services. This position requires a Bachelor’s degree in a field such as Nursing, Human Services or Social Work from an accredited institution, while many positions require a Master’s degree in these fields. In addition to education, this position often requires at least one year of experience in direct care for mentally- or developmentally-disabled patients, as well as training within social work, counseling, occupational therapy and other specialties within the field.
