Job Description for Nurse Home Care

Home care nurses provide a variety of health services to patients inside their homes; this position is typically associated with senior citizens and chronically-ill patients with complex medical needs. These nurses work primarily within their patients’ homes in all sorts of shifts, which may include overnight or weekend shifts depending on patients and their individual needs. Although they typically work independently inside homes, small teams of health care professionals may assist in the care of patients.

  • Coordinate and perform patient care activities with pharmacy, physician, home health agencies and referral source.
  • Assist+C233 with diagnostic, therapeutic and routine nursing procedures.
  • Visit patients in the home to perform technical procedures, infusion, patient assessment, patient education and other nursing duties.
  • Supervise and coordinate home health care staff.
